Requirements for registering vital events, applying for a marriage licence, and requesting certified municipal records.

Birth and death registration

  • Birth: Municipal Form 102; parents’ marriage certificate if married; and an affidavit to use the father’s surname when the parents are unmarried.
  • Death: Municipal Form 103, or Form 103A for fetal death; burial permit; and a certificate of transfer of cadaver if burial is outside Malasiqui.

Marriage licence

  • Municipal Form 97, PSA birth certificates, CENOMAR, community tax certificates and proof of pre-marriage counselling.
  • Both applicants must appear. Parental consent is required at ages 18–20; parental advice applies at ages 21–24.
  • Additional documents apply to widowed, annulled, divorced or foreign applicants.

The municipal guide lists a 10-day posting period and a ₱150 licence fee after document review.

Delayed birth registration

  • PSA negative certification and affidavits from two disinterested persons.
  • Statement on the mother’s whereabouts when applicable, parents’ marriage certificate if married, and community tax certificate.
  • At least two supporting records, such as a baptismal certificate, Form 137, insurance record, parents’ tax record or medical record.

Certified copies and legitimation

A certified municipal birth or death record is searched in the local registry after a request form is filed.

  • Certified copy: the published local fee is ₱100; record search and certification are commonly completed in about 10 minutes when the record is available.
  • Legitimation: bring the birth record, parents’ marriage record or advisory, CENOMAR, community tax certificate and affidavits from two disinterested persons.

Legitimation is listed at ₱100 with a published processing window of three to five days.
